New Patient Registration

registrationIf you would like to register with the surgery, please speak to the receptionist. In order to register at the practice you will need to provide two proofs of address to ensure you live within our catchment area (see maps). We also require photo ID to prove your identity.

If you are from an EU country you will have to provide an E112 or prove residency. If you are from a non-EU country you will need to provide your passport with valid visa.


Practice Area

Our catchment areas are people living within E9 (for the Wick Health Centre) and E5 (for 28 Median Road Surgery).


Change of Details

It is important that the Practice has up-to-date details for your. You can help us by letting us know if you change your names, address or telephone numbers. This is so we can give you the best possible care.

If you move outside our catchment area you will be required to find a new Doctor. This information can be found by telephoning 020 7683 4610 or by visiting a local Chemist or Library.
